Information about the S137 application forms
There are two forms that need to be completed, Part3 and Part4.
They need to be completed and returned to the Finance Officer – Chris Trotman. If you are not sure about something please contact him on 07814 580316 or alternatively the Parish Clerk – Vanessa Oliveri on 07837 745246
The first form is the application form itself S137 Pt3 App form V7 This is a docx form so will expand as it is used
The second form is for listing the cost of articles or services you are applying for S137 Pt4 App form 4a:4b V7 This is an excel file.
Once completed please save on your own device and then email to the Finance Officer rfo@miltonunderwychwood-pc.gov.uk
